You can register as a patient with Murrayfield Medical Centre if you live within our practice boundary and qualify for NHS treatment. If you need confirmation of whether you live within our boundary please ring us on 0131 337 2166
Queensferry Road to the north; Drumbrae, Meadowplace and Broomhouse Roads to the west; Calder Road, Longstone Street, Inglis Green Road and Slateford Road near Ardmillan, left along Murieston Road, continuing alongside the railway line to Haymarket, bypassing Dalry.
To register as a patient with us we need you to complete two forms for each person registering. The first is a “NHS Scotland Application to Register with a GP Surgery”
the second a “New Patient Health Check” questionnaire.
Once completed, please bring both forms, in person, to the surgery along with your medical card if you have it.
When you come to register we will need to see proof of identity and address so please bring a passport, bank statement or utility bill or any official document that shows your name and current address (if in doubt about acceptable documents 0131 337 2166). The documents you bring must be originals and for proof of address, no more than three months old.
When you are here, Reception staff will arrange a “New Patient Health Check” appointment for you with one of our Practice Nurses.
On submission of your application form you will be entitled to see a doctor or nurse if you need to. Your registration application will be passed for formal acceptance by a GP, and on confirmation of registration your new medical card will be sent to your home address.
When registered with us please keep your name, address and contact details up to date.

Temporary Residents
It may be possible to be seen as a temporary resident and if you do not qualify for NHS treatment we may still be able to treat you, however a fee may apply; please call at Reception or ring 0131 337 2166 and staff will advise you.
